Services: Crossways is a program provided by KershawHealth Hospice in concert with KershawHealth Home Health to provide pain, symptom management and emotional support to patients with life threatening illness still pursuing active treatment. The goal of Crossways is to provide the patient with continuity of care for their transition from Home Health to Hospice, Skilled Nursing Services, Medical Social Workers (on going bereavement support & counseling) Chaplaincy Services, Home Health Aides, Inpatient Hospice, Respite Care, Therapy Services (as deemed appropriate), Dietary and Pharmaceutical consultations and Hospice Volunteer Services |
